Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to restore function and independence
• Develop and implement individualized treatment plans focused on each resident's goals
• Collaborate with an interdisciplinary team to coordinate care and improve outcomes
• Document treatments and progress accurately and timely to meet facility and regulatory standards
• Participate in care conferences and contribute to quality improvement initiatives
• Provide compassionate, patient-centered therapy that supports safety and dignity

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical reasoning and hands-on clinical skills
• Excellent communication skills for effective team and family interactions
• Skilled nursing or post-acute experience preferred
• Dependable, flexible, and team-oriented approach to care
