Roles & Responsibilities
- Provide compassionate direct patient care and monitoring for assigned residents
- Administer medications and treatments accurately and on schedule
- Perform timely and thorough patient assessments and document findings
- Collaborate with nursing and interdisciplinary teams to coordinate care plans
- Educate residents and families on care needs and procedures in a clear, respectful manner
- Maintain accurate charting and follow facility policies and procedures
- Support safe patient transfers, wound care, and basic clinical interventions as needed
Qualifications
- Active LPN license (Licensed Practical Nurse) — must be in good standing
- Previous clinical experience in long-term care or skilled nursing preferred
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Ability to pass facility background and health screening requirements
- Strong communication skills and a team-oriented attitude
- Comfortable with electronic health record documentation
Education
- Completion of an accredited Practical Nursing program
- High school diploma or GED
Knowledge & Skills
- Solid knowledge of basic nursing principles and med administration
- Skilled in patient assessment and monitoring vital signs
- Familiar with long-term care clinical routines and resident-centered care
- Able to prioritize tasks in a fast-paced environment
- Good documentation practices and attention to detail
- Strong interpersonal skills for working with residents, families, and staff
