Roles & Responsibilities

• Provide compassionate direct nursing care to residents following care plans and facility policies
• Administer medications and treatments safely and accurately
• Perform head-to-toe assessments and report changes in condition to the care team
• Document care, observations, and interventions in the resident record in a timely manner
• Collaborate with RNs, therapists, and interdisciplinary team members to deliver holistic care
• Assist residents with activities of daily living and promote dignity and independence
• Respond to emergencies and escalate clinical concerns when needed
• Support a culture of safety, infection control, and continuous quality improvement

Qualifications

• Current, active Licensed Practical Nurse (LPN) license in Maryland required
• BLS/CPR certification required; additional certifications welcomed
• Prior experience in long-term care, skilled nursing, or rehab settings preferred (1+ years preferred)
• Proof of immunizations and background checks as required by facility
• Strong communication skills and a team-focused attitude

Education

• Graduate of an accredited practical nursing program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation
• Basic wound care and monitoring
• Resident assessment and vital sign monitoring
• Familiarity with electronic charting / EMR systems
• Infection control practices and workplace safety
• Effective communication, time management, and teamwork
