Roles & Responsibilities

• Provide compassionate, evidence-based skilled home health nursing including start-of-care, routine visits, recertifications, and discharge planning
• Deliver clinical care such as wound care, wound vac management, dressing changes, ostomy care and patient education
• Manage medications and perform medication administration (IM, subcutaneous, IV infusions as needed) with thorough medication assessments
• Start and maintain IVs, assist with PICC line care and phlebotomy as appropriate
• Perform comprehensive assessments and document thoroughly in EPIC and OASIS (accurate charting is critical)
• Coordinate care with interdisciplinary team members and provide patient/family education to support safe transitions
• Maintain infection control precautions and follow facility safety guidelines, including COVID vaccination and booster requirements

Qualifications

• Active RN license — Oregon and Washington licensure required (pending licenses may be considered; confirm status with recruiter)
• Minimum 1 year of skilled Home Health RN experience
• BCLS (Basic Cardiac Life Support) certification required
• Comfortable traveling to patient homes up to ~60 miles per day; reliable personal transportation required
• Experience with OASIS and EPIC charting preferred; please highlight OASIS experience in your application
• Current immunizations per facility policy (primary COVID series plus one booster required)

Education

• Active RN licensure (OR & WA)
• ADN or BSN preferred but not strictly required if experience meets requirements

Knowledge & Skills

• Strong wound care knowledge including wound vac management
• IV therapy, PICC line care, phlebotomy and IV starts
• Medication administration and assessment skills
• Proficiency with OASIS documentation and EPIC charting
• Excellent patient education, teaching, and communication skills
• Time management and route planning to optimize visits and minimize drive time
• Clinical judgment in home environments and ability to work independently
