Roles & Responsibilities

• Provide hands-on treatment to patients following the Physical Therapist's plan of care
• Record daily treatment notes and weekly progress notes in accordance with PT Board requirements
• Instruct and train caregivers, nursing staff, and families on maintenance programs to support discharge planning
• Communicate patient progress, problems, and plans clearly with supervisors and the interdisciplinary care team
• Participate in patient care and rehabilitation conferences and facility in-service training
• Maintain a safe, clean treatment area and report any equipment issues for timely repair

Qualifications

• Licensed Physical Therapy Assistant in the state of California (must maintain license in good standing)
• Proficient with documentation systems such as Casamba and PointClickCare
• Ability to complete and maintain required continuing education per state board practices
• Strong communication skills and ability to relate positively with patients, families, and colleagues
• Demonstrated ability to teach caregivers and nursing staff restorative/maintenance programs

Education

• Licensed Physical Therapy Assistant per state board requirements
• Must meet and maintain continuing education requirements as required by the state board

Knowledge & Skills

• Patient treatment per PT plan and hands-on therapeutic techniques
• Accurate treatment documentation and weekly progress note preparation
• Experience using Casamba and PointClickCare (or similar clinical documentation systems)
• Caregiver and nursing instruction for restorative care
• Effective interdisciplinary communication and teamwork
• Basic equipment maintenance awareness and reporting
