Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Deliver compassionate physical therapy care to patients across Sacramento County in their homes
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric diagnoses
Collaborate with interdisciplinary teams to create personalized treatment plans
Document patient progress and maintain accurate mileage records for reimbursement
Participate in twice-monthly supervisor meetings to review documentation and ensure quality care

Qualifications

Active Physical Therapy Assistant license in California
Minimum 6 months of clinical experience preferred
Strong communication and time-management skills
Ability to work independently in home health settings
Valid driver's license and reliable transportation

Knowledge & Skills

Home health documentation and productivity standards
Patient-centered care across multiple specialties
Mileage tracking and GSA-compliant reimbursement processes
Building rapport with patients and families in home environments
Meeting productivity goals of 5-6 visits per day
