Physical Therapist - Skilled Nursing Facility

Location: Santa Monica, CA

Shift Type: PRN / Per Diem; flexible shifts (days and times vary based on facility needs)

Candidate Pay Rate: $58 - $63 per hour

Roles & Responsibilities

Perform thorough evaluations and develop individualized physical therapy care plans
Deliver therapeutic exercises and functional mobility training to support recovery
Educate residents and caregivers on safe techniques and home programs
Collaborate with nursing and the interdisciplinary team to coordinate care
Maintain accurate, compliant documentation to support patient outcomes

Qualifications

Active California Physical Therapy license required
Experience in skilled nursing facility or geriatric rehabilitation preferred
Strong clinical reasoning and hands-on therapy skills
Excellent communication skills for working with residents, families, and team members
Comfortable working in a PRN / per diem capacity with flexible scheduling

Knowledge & Skills

Skilled in patient evaluation and individualized treatment planning
Proficient with therapeutic exercise and functional mobility training
Knowledgeable about documentation best practices and regulatory compliance
Ability to educate residents and caregivers in a clear, compassionate way
Experience collaborating within an interdisciplinary care team
Familiarity with geriatric rehabilitation principles
