Physical Therapist - Skilled Nursing Facility
Location: Santa Monica, CA
Shift Type: PRN / Per Diem; flexible shifts (days and times vary based on facility needs)
Candidate Pay Rate: $58 - $63 per hour
Roles & Responsibilities
- Perform thorough evaluations and develop individualized physical therapy care plans
- Deliver therapeutic exercises and functional mobility training to support recovery
- Educate residents and caregivers on safe techniques and home programs
- Collaborate with nursing and the interdisciplinary team to coordinate care
- Maintain accurate, compliant documentation to support patient outcomes
Qualifications
- Active California Physical Therapy license required
- Experience in skilled nursing facility or geriatric rehabilitation preferred
- Strong clinical reasoning and hands-on therapy skills
- Excellent communication skills for working with residents, families, and team members
- Comfortable working in a PRN / per diem capacity with flexible scheduling
Knowledge & Skills
- Skilled in patient evaluation and individualized treatment planning
- Proficient with therapeutic exercise and functional mobility training
- Knowledgeable about documentation best practices and regulatory compliance
- Ability to educate residents and caregivers in a clear, compassionate way
- Experience collaborating within an interdisciplinary care team
- Familiarity with geriatric rehabilitation principles
