Candidate Pay Rate: $150 - $105 per hour

Roles & Responsibilities

Evaluate patients and create individualized physical therapy treatment plans that promote mobility and independence
Deliver targeted therapeutic exercises, gait training, and adaptive strategies to improve function
Monitor patient progress, adjust interventions as needed, and celebrate functional gains
Collaborate closely with physicians, nursing, and rehab staff to coordinate patient-centered care
Maintain accurate, timely clinical documentation to support continuity of care and outcomes
Educate patients and families on home programs and safe mobility techniques

Qualifications

Active California Physical Therapist License required
Minimum 1 year of Physical Therapy experience
Previous SNF or rehabilitation experience preferred
Current BLS certification required
Strong clinical assessment skills and a passion for geriatric care

Knowledge & Skills

Skilled in clinical assessment, therapeutic exercise, and gait training
Experience with care planning and progress documentation
Ability to collaborate with interdisciplinary teams and communicate effectively
Knowledge of mobility, balance, strength training, and functional independence strategies
Comfort working in a skilled nursing facility environment and supporting geriatric patients
