Roles & Responsibilities

• Assess patients' physical abilities and functional limitations using evidence-based evaluation techniques
• Develop individualized treatment plans with clear, measurable goals
• Implement therapeutic interventions including therapeutic exercise, manual therapy and functional training
• Document progress and communicate changes in condition in a timely, professional manner
• Collaborate with interdisciplinary team members to coordinate high-quality patient care
• Educate patients and families on home programs, safety and strategies to maximize independence
• Contribute to a positive clinic culture by mentoring colleagues and participating in team initiatives

Qualifications

• Class: PT (Physical Therapist) — licensed or eligible to be licensed as a Physical Therapist in the state of practice
• Current basic life support (BLS) certification preferred or able to obtain
• Clinical proficiency in patient evaluation and therapeutic intervention
• Strong communication and documentation skills
• Ability to work as part of a multidisciplinary rehabilitation team

Education

• Graduation from an accredited Physical Therapy program
• Doctor of Physical Therapy (DPT) preferred where applicable

Knowledge & Skills

• Solid foundation in physical therapy assessment and treatment planning
• Skilled in therapeutic exercise, manual therapy and gait training
• Proficient documentation practices and working knowledge of clinical charting
• Comfortable collaborating with physicians, nurses and rehab staff
• Patient education and family-centered care skills
