Roles & Responsibilities
- Conduct thorough physical therapy evaluations to guide patient recovery
- Establish personalized plans of care that make real differences
- Collaborate with rehab team through frequent communication on progress and needs
- Assist with rounds and care conferences to ensure seamless patient journeys
- Deliver treatments in rooms, hallways, outdoor areas, and state-of-the-art rehab gym
- Float between acute, swing, nursing home, and outpatient services as needed
Qualifications
- Active NC Physical Therapy license
- CPR certification
- At least 2 years of experience, preferably 3+ in hospital or nursing home settings
Education
- Physical Therapy degree with NC licensure
Knowledge & Skills
- Hospital and nursing home rehab experience
- Proficiency with Cerner and NetHealth documentation
- Strong team communication for patient progress and discharge planning
- Adaptability across acute, swing bed, nursing home, and outpatient services
