Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to promote recovery and independence
• Develop and implement individualized treatment plans that help residents reach their highest functional potential
• Collaborate with an interdisciplinary team to ensure coordinated, patient-centered care
• Document evaluations, treatment plans, and progress accurately and timely in accordance with facility and regulatory standards
• Participate in care conferences and contribute to quality improvement initiatives that enhance resident outcomes

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical reasoning and hands-on PT skills
• Excellent communication skills for interacting with residents, families, and the care team
• Skilled nursing or post-acute care experience preferred
• Dependable, flexible, and team-oriented approach to patient care
• Experience with clinical documentation and regulatory compliance
