Roles & Responsibilities

• Assess patients and develop individualized outpatient physical therapy treatment plans
• Deliver hands-on and therapeutic interventions to help patients meet functional goals
• Document clinical care thoroughly and timely using required clinical documentation standards
• Collaborate with clinic staff and interdisciplinary teams to coordinate patient care
• Support quality initiatives and maintain compliance with licensing and billing requirements
• Work with NCH credentialing and provider enrollment to ensure you are approved to see patients

Qualifications

• Minimum 2 years of current outpatient physical therapy experience (required)
• Active New Hampshire Physical Therapy license (Compact license holders eligible)
• Active American Heart Association BLS (required)
• License verification and up-to-date certifications at time of submission
• Provider enrollment must be completed prior to seeing patients (credentialing may take 60+ days)
• One supervisory signed reference from the past year of employment (required)
• Previous travel experience highly preferred

Knowledge & Skills

• Strong outpatient physical therapy and patient care skills
• Clinical documentation proficiency and attention to detail
• Familiarity with provider enrollment processes and NPI requirements
• Ability to work flexible schedules and adapt to changing clinic needs
• Effective communication and teamwork in a clinical setting
• Comfort working with varied patient populations in an outpatient clinic
