Roles & Responsibilities

• Evaluate patients and determine individualized treatment needs using evidence-based assessment
• Develop clear, goal-driven treatment plans to restore strength, motion and function
• Implement therapeutic procedures and modalities to manage pain and improve mobility
• Monitor patient progress and adjust plans to maximize outcomes
• Document thoroughly and accurately in accordance with regulations and facility standards
• Collaborate with referring physicians and the interdisciplinary care team to coordinate care
• Educate patients and families to promote recovery, self-management and long-term wellness
• Ensure all practice aligns with the California State Practice Act and facility policies

Qualifications

• Current CA Physical Therapist License (required) — active and in good standing
• Ability to develop and implement treatment programs using a variety of therapeutic modalities
• Strong documentation skills and familiarity with regulatory requirements
• Professional communication skills and a team-oriented mindset
• Commitment to patient-centered care and evidence-based practice
