Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients in their place of residence
Develop and implement individualized treatment plans to improve mobility and function
Educate patients and families on therapeutic exercises and home safety
Collaborate with interdisciplinary team to ensure holistic patient care
Accurately document patient progress using HomeCare/HomeBase EMR
Travel within a 60-mile radius to deliver compassionate care

Qualifications

Current North Dakota Physical Therapy License
BLS certification from the American Heart Association
Valid driver's license and auto insurance
Minimum 1 year of home health experience
Previous travel experience is not required – we welcome first-time travelers

Knowledge & Skills

Skilled in treating post-surgical, fall-related, and hospice patients
Proficient with HomeCare/HomeBase EMR system
Ability to work independently with minimal supervision
Strong communication and patient education skills
