Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients in Tehama County
Conduct initial assessments and develop individualized care plans
Perform SOC (start of care) visits for orthopedic patients when nursing is not ordered
Collaborate with interdisciplinary team including RNs and therapists
Educate patients and families on therapeutic exercises and safety
Document patient progress accurately using HCHB (HomeCare HomeBase) EMR

Qualifications

Active Physical Therapy license in California (or compact eligible)
Minimum 1 year of home health experience (travelers with less than 2 years considered)
Current BLS certification (American Heart Association)
Strong communication and organizational skills
Ability to travel within Tehama County (approx. 40 miles/day)

Education

Graduate of an accredited Physical Therapy program (Master's or Doctorate)

Knowledge & Skills

Home health therapy best practices and Medicare guidelines
Competency in HCHB or similar home health EMR
Experience with orthopedic and geriatric patient populations
Time management and autonomy in a field-based role
Compassionate patient-centered care approach
