Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct start of care, recertifications, and evaluations for home health patients
Provide physical therapy services to patients with CHF, orthopedic conditions, pneumonia, and other diagnoses
Collaborate with clinical manager and educator for guidance and support
Maintain accurate documentation and communicate effectively with interdisciplinary team

Qualifications

Valid Physical Therapist (PT) license in Kentucky
BLS certification required
Home health experience preferred but not required, new grads welcome to apply
Ability to work rotating weekends and holidays

Knowledge & Skills

Expertise in home health physical therapy
Knowledge of conditions such as CHF, orthopedic issues, pneumonia
Strong clinical assessment and documentation skills
Excellent communication and patient education skills
