Roles & Responsibilities

Provide compassionate hands-on nursing care to residents and patients
Administer medications and treatments accurately and safely
Monitor and document vital signs, patient status, and clinical changes
Collaborate with RNs and interdisciplinary team members to deliver coordinated care
Maintain clear, timely electronic and paper charting for continuity of care
Support infection control practices and maintain a clean, safe environment
Educate patients and families about care plans and health maintenance

Qualifications

Current, active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain prior to start)
Ability to pass background check and meet facility onboarding requirements
Comfortable working in a resident/patient-focused setting and collaborating with a care team
Prior experience in long-term care or skilled nursing preferred but not required

Education

Graduate of an accredited Practical Nursing program; LPN diploma or equivalent required

Knowledge & Skills

Medication administration and safe documentation practices
Basic clinical assessment and vital sign monitoring
Familiarity with electronic medical records and charting best practices
Strong communication and interpersonal skills with patients and families
Time management and the ability to prioritize patient needs
