Roles & Responsibilities
- Provide compassionate direct patient care within your scope as an LPN
- Administer medications and treatments accurately and safely
- Monitor, record, and report patient status and changes to the care team
- Assist with patient hygiene, mobility, and comfort to improve outcomes
- Collaborate with RNs and interdisciplinary staff to coordinate excellent care
- Follow infection control and safety protocols to protect patients and staff
- Document nursing care in the electronic health record in a timely manner
Qualifications
- Active LPN license in Pennsylvania (required)
- Completion of an accredited LPN/LVN program
- Current Basic Life Support (BLS) certification or ability to obtain upon hire
- Ability to pass background and health screenings per facility policy
- Strong communication skills and a patient-centered attitude
- Previous experience in long-term care or skilled nursing preferred
Education
- Graduate of an accredited LPN/LVN program
- High school diploma or GED
Knowledge & Skills
- Medication administration and basic wound care
- Patient assessment and vital signs monitoring
- Familiarity with electronic health records and documentation best practices
- Infection control procedures and safety standards
- Team collaboration, time management, and prioritization skills
- Compassionate patient interaction and family communication
