Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and document thoroughly
Monitor vital signs, observe changes, and report concerns promptly
Collaborate with the interdisciplinary care team to support individualized care plans
Maintain accurate and timely electronic documentation and handoffs
Support resident activities of daily living with dignity and respect
Mentor and support CNAs and other team members as needed

Qualifications

Licensed Practical Nurse (LPN) — this role is for LPN-level clinicians
Active LPN license appropriate for practice in Pennsylvania (PA)
Current Basic Life Support (BLS/CPR) certification (or willingness to obtain)
Comfortable working in long-term care/skilled nursing environments
Strong communication skills and a team-oriented mindset

Education

LPN/Vocational Nursing diploma or equivalent completion of an accredited LPN program

Knowledge & Skills

Medication administration and safe documentation practices
Basic wound care and vital sign assessment
Experience with electronic medical record (EMR) documentation
Ability to prioritize tasks and respond calmly in clinical situations
Patient-centered approach and strong interpersonal skills
