Roles & Responsibilities
- Provide compassionate direct patient care consistent with LPN scope of practice
- Administer medications and treatments accurately and document per policy
- Monitor patients’ conditions and report changes to the care team promptly
- Assist with care planning and implement delegated nursing tasks
- Maintain clear, timely charting and help ensure accurate handoffs
- Collaborate with nurses and interdisciplinary staff to support excellent outcomes
Qualifications
- Active LPN license in Pennsylvania (required)
- Current BLS/CPR certification (required or obtain upon hire)
- Completion of an accredited practical nursing program
- Comfortable with medication administration, basic wound care, and charting
- Successful completion of background check and any facility onboarding requirements
- Prior long-term care or clinical experience preferred but new grads considered
Education
- Graduate of an accredited Practical Nursing (LPN/LVN) program
- High school diploma or GED
Knowledge & Skills
- Strong medication administration skills and knowledge of safe practices
- Basic wound care and vital signs assessment
- Clear communication skills with patients, families, and the care team
- Familiarity with electronic health records and accurate documentation
- Ability to prioritize tasks in a busy clinical environment
- Professionalism, reliability, and a patient-centered attitude
