Roles & Responsibilities

Provide compassionate direct nursing care to residents, focusing on safety and dignity
Administer medications and treatments accurately and document per facility protocols
Monitor patients’ condition, report changes to RN/physician, and follow care plans
Perform wound care, ADL assistance, and other skilled nursing tasks as assigned
Communicate clearly and professionally with residents, families, and care team members
Participate in shift handovers and maintain thorough, timely documentation

Qualifications

Current LPN license valid in Pennsylvania (required)
Basic Life Support (BLS) / CPR certification (or willingness to obtain)
Ability to pass background check, drug screen, and required immunizations
Strong clinical judgment, attention to detail, and friendly bedside manner
Previous long-term care or skilled nursing experience preferred but not required

Education

LPN/Vocational Nursing diploma or certificate (required)

Knowledge & Skills

Medication administration and accurate charting
Familiarity with basic nursing assessments and vital sign monitoring
Infection control practices and wound care basics
Clear communication and teamwork in a fast-paced environment
Time management and ability to prioritize patient needs
