Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments accurately and safely
- Monitor patient vital signs and report changes promptly to the care team
- Collaborate with RNs and interdisciplinary staff to implement care plans
- Document patient care, observations, and interventions in charts/EHR
- Support resident comfort, dignity, and activities of daily living
- Participate in safety, infection control, and quality improvement efforts
Qualifications
- Current LPN license in Pennsylvania (or compact/state-appropriate) — required
- CPR/BLS certification — current and active
- Prior LPN experience in long-term care, skilled nursing, or similar clinical setting preferred
- Strong communication and teamwork skills; patient-centered mindset
- Ability to pass background checks and meet facility onboarding requirements
Education
- Graduate of an accredited Practical Nursing (LPN) program
- High school diploma or equivalent
Knowledge & Skills
- Medication administration and documentation best practices
- Basic and focused patient assessment skills
- Familiarity with infection control and safety protocols
- Comfortable charting and maintaining accurate patient records
- Ability to work efficiently in a fast-paced clinical environment
