Roles & Responsibilities
- Provide compassionate bedside care and support to residents/patients
- Administer medications and treatments safely and accurately
- Monitor and record vital signs, reporting changes to the care team
- Document patient care and progress clearly in the electronic record
- Assist with activities of daily living and encourage patient independence
- Communicate effectively with nurses, providers, and families
- Follow infection control and safety protocols to protect patients and staff
- Contribute to a positive care environment and participate in team meetings
Qualifications
- Valid LPN (Licensed Practical Nurse) license in Pennsylvania (LPN/LVN)
- Current BLS/CPR certification (or willing to obtain promptly)
- Strong communication skills and a patient-centered attitude
- Ability to pass background check and drug screening as required
- Reliable, professional, and comfortable working as part of a care team
Education
- Diploma or certificate from an accredited Practical Nursing program (LPN)
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic patient assessment and monitoring of vital signs
- Charting in electronic medical records and accurate recordkeeping
- Infection control, safety procedures, and standard precautions
- Patient mobility assistance and support with ADLs
- Teamwork, time management, and effective communication
