Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care, monitoring vitals and reporting changes
Administer medications and treatments according to facility policies and provider orders
Maintain accurate and timely documentation in the patient record
Perform wound care and routine clinical procedures within LPN scope
Collaborate with RNs and interdisciplinary team members to coordinate patient care
Educate patients and families on care plans, medications, and follow-up instructions
Support a safe, clean environment and adhere to infection control standards

Qualifications

Valid LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain upon hire)
Clinical experience in a long-term care or skilled nursing setting preferred
Strong communication and teamwork skills
Ability to pass required background checks and health screenings

Education

LPN diploma, certificate, or equivalent clinical training required

Knowledge & Skills

Medication administration and basic IV/drug knowledge within LPN scope
Vital signs assessment and patient monitoring
Wound care and basic clinical procedures
Familiarity with electronic medical records and charting
Infection prevention and safety protocols
Patient education and family communication skills
