Roles & Responsibilities
- Provide compassionate bedside care to residents and patients consistent with LPN scope of practice
- Administer medications and treatments accurately and safely
- Monitor and document vital signs, patient status, and changes in condition
- Accurately complete charting and electronic medical records in a timely manner
- Collaborate with RNs, CNAs, and interdisciplinary team members to support care plans
- Assist with daily living activities and promote a respectful, dignified environment
- Communicate effectively with patients, families, and team members to support positive outcomes
Qualifications
- Current LPN license (Pennsylvania) required
- CPR/BLS certification required or obtained upon hire
- Comfortable working 8-hour clinical shifts in a long-term care or similar setting
- Strong communication and interpersonal skills
- Reliable, patient-focused, and able to work as part of a team
Education
- LPN diploma or equivalent from an accredited program
Knowledge & Skills
- Medication administration and documentation
- Basic patient assessment and monitoring
- Familiarity with wound care and simple dressing changes
- Charting in electronic medical records and accurate documentation practices
- Ability to prioritize tasks and handle multiple patient needs
