Roles & Responsibilities
- Provide compassionate direct patient care, including vitals, medication administration, and wound care
- Collaborate with the interdisciplinary team to create and follow individualized care plans
- Monitor patients and report changes in condition promptly to nursing leadership and providers
- Document care accurately and timely in the patient record to ensure continuity
- Support residents in activities of daily living with dignity and respect
- Assist in maintaining a safe, clean, and therapeutic environment for patients and colleagues
Qualifications
- Active LPN licensure or eligible for licensure in Pennsylvania
- Current BLS/CPR certification (or ability to obtain promptly)
- Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
- Excellent communication and teamwork skills
- Ability to follow established clinical protocols and maintain accurate documentation
Education
- Graduate of an accredited practical nursing (LPN) program
Knowledge & Skills
- Strong clinical assessment and basic nursing skills
- Medication administration knowledge and safe practice
- Familiarity with infection control and patient safety principles
- Comfortable using electronic health records and basic computer skills
- Ability to prioritize, multitask, and stay composed in a fast-paced environment
