Roles & Responsibilities

• Provide compassionate direct patient care, putting patients first every shift
• Administer medications and treatments safely and accurately according to orders
• Monitor, assess and document patient vitals and changes in condition
• Communicate clearly and professionally with patients, families, and the care team
• Assist with care planning, patient education, and discharge preparations
• Maintain accurate and timely electronic and paper documentation
• Adhere to infection control standards and facility policies to keep everyone safe
• Collaborate with interdisciplinary team members to deliver high-quality care

Qualifications

• Active LPN license in Pennsylvania required
• Completion of an accredited LPN/LVN program
• Current CPR/BLS certification required or obtained upon hire
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Strong communication skills, teamwork mindset, and reliable attendance
• Ability to pass background check and drug screening as required by facility

Education

• Completion of an accredited LPN/LVN program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation
• Basic wound care and activities of daily living support
• Patient assessment, vital signs monitoring, and recognition of changes
• Familiarity with electronic health records and charting
• Time management, prioritization, and multitasking under pressure
• Professional communication with patients, families, and clinicians
