Roles & Responsibilities

• Provide high-quality direct patient care with compassion and attention to detail
• Administer medications and treatments safely and accurately
• Monitor and record patient vital signs, reporting significant changes promptly
• Collaborate with the interdisciplinary team to create and follow care plans
• Perform wound care, catheter care, and other clinical procedures as required
• Accurately document patient interactions and clinical observations in the chart
• Support patients and families with clear communication and empathy

Qualifications

• Licensed Practical Nurse (LPN) credential required
• Current, active LPN license in Pennsylvania (or compact privileges if applicable)
• Current Basic Life Support (BLS) certification required
• Previous hands-on clinical experience preferred (long-term care, skilled nursing, or similar settings helpful)
• Clear background check and ability to meet facility onboarding requirements
• Strong communication skills and a patient-centered approach

Education

• Completion of an accredited LPN/LVN program or equivalent vocational training

Knowledge & Skills

• Medication administration and charting best practices
• Basic assessment skills: vitals, intake/output, skin/wound checks
• Familiarity with electronic medical records and documentation standards
• Infection control procedures and safe clinical practice
• Time management and the ability to prioritize patient needs
• Team collaboration and clear interpersonal communication
