Roles & Responsibilities

• Provide compassionate, routine nursing care under the direction of supervising RN or management
• Administer medications and treatments accurately and document according to policy
• Monitor resident health status and report changes promptly to the care team
• Assist with activities of daily living while promoting dignity and independence
• Communicate clearly and professionally with residents, families, and staff
• Maintain accurate, timely documentation in resident charts and electronic records
• Participate in team huddles and contribute to a positive, collaborative care environment

Qualifications

• Active LPN license in Pennsylvania or eligible for state licensure
• Completion of an accredited practical nursing program
• Current Basic Life Support (BLS) certification or willingness to obtain
• Previous experience in long-term care, skilled nursing, or similar setting preferred
• Strong communication skills and the ability to work effectively as part of a care team
• Reliable, professional, and patient-centered approach to care

Education

• High school diploma or equivalent
• Completion of an accredited LPN / practical nursing program

Knowledge & Skills

• Solid understanding of medication administration and basic nursing procedures
• Familiarity with documentation standards and electronic health records
• Ability to assess resident needs and escalate concerns appropriately
• Good time management and prioritization skills during busy shifts
• Compassionate bedside manner and strong interpersonal skills
