Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Perform thorough assessments and communicate changes in condition promptly
• Administer medications and treatments accurately and safely
• Document patient care and observations clearly in the electronic chart
• Collaborate with RNs, CNAs and interdisciplinary team members to coordinate care
• Support resident dignity and daily living activities with respect and empathy

Qualifications

• Active LPN license in Pennsylvania (must hold and maintain)
• Graduation from an accredited LPN program or equivalent practical nursing education
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfortable administering medications and performing clinical procedures within scope
• Ability to pass background check and meet facility immunization requirements
• Previous long-term care or clinical experience preferred but enthusiastic new grads encouraged to apply

Education

• LPN diploma or certificate from an accredited program
• High school diploma or GED

Knowledge & Skills

• Strong patient assessment and vital sign monitoring skills
• Medication administration and documentation accuracy
• Familiarity with electronic health records and charting
• Excellent communication and teamwork with clinical staff and families
• Time management and prioritization during busy shifts
• Infection control practices and safety-focused care
