Contract Type: Per-Diem

Shift Type: Per diem, 8.5-hour shift; schedule varies (day and/or evening shifts possible). Specific start/end times and days will be assigned when scheduled.

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, high-quality nursing care to residents as an LPN
Administer medications and document administration according to facility policy
Monitor resident condition, record vital signs, and report changes to the care team
Support activities of daily living and promote resident comfort and dignity
Collaborate with RNs, CNAs, and interdisciplinary teams to ensure continuity of care
Maintain accurate records and complete timely charting and shift handoffs
Adhere to infection control practices and facility protocols to keep residents safe

Qualifications

Current, active LPN license in Pennsylvania (required)
BLS/CPR certification required or willingness to obtain promptly
Prior experience in long-term care or skilled nursing preferred
Ability to pass background check and drug screen as required by the facility
Strong communication skills and a team-oriented mindset

Education

Completion of an accredited Practical Nursing program and high school diploma or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care, infection control, and vital sign assessment
Resident assessment and recognition of changes in condition
Electronic or paper charting and accurate record keeping
Effective communication with residents, families, and the clinical team
