Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Hopkins Center
• Administer medications and treatments accurately and on time
• Monitor, assess, and document patient status and changes in condition
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Support admissions, transfers, and discharges with professionalism and attention to detail
• Educate residents and families about care plans and safety practices
• Maintain accurate electronic medical records and comply with facility policies

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of practice
• Current BLS/CPR certification or willingness to obtain prior to start
• Prior experience in long-term care or similar clinical setting preferred
• Strong communication skills and a team-first mindset
• Clear background checks and required health screenings for facility credentialing

Education

• Completion of an accredited Practical Nursing program or equivalent

Knowledge & Skills

• Medication administration and documentation
• Basic wound care and infection control practices
• Vital signs monitoring and clinical assessment
• Familiarity with electronic medical record charting
• Ability to prioritize tasks in a fast-paced care environment
• Patient and family education with empathy and clarity
