Roles & Responsibilities

• Provide skilled occupational therapy services to residents in a skilled nursing facility, focusing on improving independence and quality of life
• Assess functional abilities and develop individualized treatment plans based on clinical evaluation
• Implement evidence-based interventions and adaptive techniques to promote daily living skills
• Document treatment progress clearly and timely in the facility’s records
• Collaborate with interdisciplinary team members and families to coordinate care and discharge planning
• Educate residents and caregivers on home programs, adaptive equipment, and safety strategies
• Advocate for residents’ needs while maintaining a compassionate, patient-centered approach

Qualifications

• Certified Occupational Therapy Assistant (COTA) credential (required)
• Eligible for or holding state licensure/registration as required by Indiana (ability to obtain if not currently licensed)
• Comfortable working in a Skilled Nursing Facility (SNF) environment
• Strong communication skills and ability to collaborate with an interdisciplinary team
• New grads welcome — the role accepts clinicians with any experience level, including new graduates
• Current CPR/BLS certification (or willingness to obtain) as per facility policy

Education

• Associate degree in Occupational Therapy Assistant (OTA) or equivalent

Knowledge & Skills

• Familiarity with SNF care processes and geriatric rehabilitation needs
• Skilled in ADL/IADL training, transfers, and mobility interventions
• Competent documentation skills and basic comfort with electronic health records
• Ability to assess and recommend adaptive equipment and home modifications
• Good clinical judgment, time management, and patient-centered clinical reasoning
