Roles & Responsibilities

• Provide compassionate direct patient care to residents, ensuring dignity and comfort
• Administer medications and treatments according to orders and facility protocols
• Assess and monitor changes in resident conditions and communicate promptly to the care team
• Accurately document care, observations, and patient responses in the medical record
• Collaborate with nursing staff and interdisciplinary team to support individualized care plans
• Support resident and family education, answering questions with clarity and empathy
• Assist with safe transfers, activities of daily living, and infection prevention practices

Qualifications

• Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) credential
• Able to work the assigned schedule: Monday through Wednesday, 5:00 AM - 5:30 PM
• Comfortable working in a skilled nursing/rehab setting and collaborating with an interdisciplinary team

Education

• Graduate of an accredited LPN or RN program appropriate to the license held

Knowledge & Skills

• Strong clinical assessment and observation skills
• Medication administration and basic wound care knowledge
• Accurate charting and documentation practices
• Clear communication and teamwork with nursing and rehab staff
• Resident-centered care and safety-focused practice
