Roles & Responsibilities

• Perform thorough evaluations to establish individualized treatment plans
• Deliver daily hands-on treatments focused on returning residents home
• Manage safe and efficient discharges that support patient goals
• Document progress clearly and timely to meet facility and payer requirements
• Collaborate with the interdisciplinary rehab team to optimize patient outcomes
• Maintain productivity standards while delivering compassionate care

Qualifications

• Licensed Physical Therapist (PT) — DPT credentials encouraged and accepted
• 1-3 years clinical experience preferred (prior SNF experience strongly preferred)
• Comfortable performing evaluations, daily treatments, and discharges in a SNF setting
• Ability to follow facility policies and productivity expectations (90% productivity noted)
• Willingness to work onsite in black scrubs and adhere to employer dress code

Education

• DPT preferred or equivalent PT license/certification required

Knowledge & Skills

• Skilled Nursing Facility (SNF) rehab workflows and documentation
• Skilled evaluation and treatment planning for geriatric and post-acute populations
• Safe transfer and mobility training techniques
• Strong communication and teamwork with nursing and rehab staff
• Time management skills to meet productivity and scheduling needs
