Roles & Responsibilities

• Evaluate residents to determine speech, language, cognitive-communication and swallowing needs
• Develop and implement individualized treatment plans that help residents meet goals
• Provide skilled therapy sessions focused on safe swallowing, communication and functional outcomes
• Document progress and maintain clear, timely clinical notes in facility records
• Collaborate with nursing, rehab team members and families to coordinate resident-centered care
• Participate in care planning and discharge/transition planning as needed

Qualifications

• Active SLP credentials and licensure (ability to work in Ohio preferred; facility requirements will apply)
• 1-3 years of clinical experience working as an SLP (SNF/geriatric experience a plus)
• Strong documentation skills and familiarity with skilled nursing facility processes
• Comfortable evaluating and treating dysphagia, cognitive-communication and voice disorders
• Professional communication skills and ability to collaborate with interdisciplinary teams

Knowledge & Skills

• Skilled in clinical evaluation and treatment planning for adult/geriatric populations
• Knowledge of SNF workflows, MDS and care-plan coordination
• Competence in dysphagia assessment and safe swallowing strategies
• Clear clinical documentation and progress tracking
• Teamwork, patient advocacy and family education skills
