Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Support safe transfers, mobility assistance, and fall prevention strategies
• Maintain accurate, timely documentation of care and resident responses
• Adhere to infection control standards and facility policies to protect residents
• Work collaboratively with nurses and therapy staff to deliver person-centered care

Qualifications

• Active Certified Nursing Assistant (CNA) certification (state recognition required)
• Comfortable working a 6:00 AM - 6:30 PM shift and providing hands-on resident care
• Ability to pass required background checks, drug screening, and facility onboarding
• Strong communication skills and a team-first attitude
• Previous long-term care or rehab experience is a plus but not required

Education

• High school diploma or equivalent
• Completion of an accredited CNA program and current CNA certification

Knowledge & Skills

• Basic clinical skills: vital signs, ADL assistance, safe transfers
• Knowledge of infection control and standard precautions
• Accurate documentation and charting practices
• Compassionate resident interaction and strong interpersonal skills
• Ability to follow care plans and report clinical changes clearly
