Roles & Responsibilities

• Conduct comprehensive speech-language evaluations for residents
• Provide daily treatments that improve communication and swallowing function
• Manage timely and safe patient discharges with clear plans
• Collaborate with the interdisciplinary therapy team to optimize patient outcomes
• Maintain accurate documentation and meet facility productivity expectations
• Follow Continuum Rehabilitation Group policies and facility procedures

Qualifications

• Valid SLP licensure for the state of New Mexico (or ability to obtain prior to assignment)
• 1-3 years of SLP clinical experience (as listed)
• Prior SNF experience strongly preferred
• Comfortable working on-site and adhering to facility dress code (black scrubs)
• Ability to meet a productivity requirement (90% as noted in the posting)

Knowledge & Skills

• Skilled in speech-language evaluations
• Experience delivering daily therapeutic treatments
• Knowledgeable in discharge planning and transitions of care
• Familiarity with SNF practice patterns and documentation standards
• Strong teamwork, communication, and time-management skills
• Comfortable with productivity tracking and meeting performance goals
