Roles & Responsibilities

• Provide skilled physical therapy services to residents in a skilled nursing facility
• Perform thorough assessments and create individualized treatment plans that drive functional gains
• Deliver hands-on interventions such as gait training, balance work, and mobility restoration
• Document evaluations, progress notes, and discharge plans in accordance with facility policies
• Collaborate closely with nurses, CNAs, and the interdisciplinary team to coordinate care
• Meet productivity expectations and clinical quality standards while keeping patient-centered care front and center
• Adhere to CRG policies and the facility dress code and be present on-site to receive guaranteed hours

Qualifications

• Licensed Physical Therapist in New Mexico or ability to obtain licensure prior to start
• DPT or equivalent PT degree
• 1 to 3 years of physical therapy experience, ideally with skilled nursing facility patients
• Comfortable following organizational policies and productivity expectations
• Professional demeanor, strong communication skills, and ability to work as part of a team
• Willingness to wear black scrubs as required by the facility

Education

• DPT preferred; PT degree or equivalent required for licensure

Knowledge & Skills

• Understanding of skilled nursing facility workflows and regulatory expectations
• Skilled in patient assessment, treatment planning, and functional mobility interventions
• Strong clinical documentation skills and familiarity with productivity tracking
• Ability to collaborate with interdisciplinary teams and communicate with patients and families
• Adaptability to a variety of patient diagnoses typical in SNF settings
