Candidate Pay Rate: $28 per hour

Roles & Responsibilities

Provide compassionate direct patient care and monitor patient status throughout your shift
Administer medications and treatments accurately and safely per physician orders
Document assessments, interventions, and patient responses in the medical record
Collaborate with RNs, providers, and support staff to develop and update care plans
Educate patients and families on care plans, medications, and discharge instructions
Assist with admissions, transfers, and safe patient handoffs to ensure continuity

Qualifications

Current Licensed Practical Nurse (LPN) license (state licensure required)
CPR/BLS certification (or willingness to obtain)
Prior experience in long-term care or similar clinical setting preferred
Strong clinical assessment, medication administration, and documentation skills
Clear background check and ability to meet facility onboarding requirements

Education

Vocational Nursing diploma or equivalent LPN training program

Knowledge & Skills

Solid knowledge of medication administration and basic nursing procedures
Comfortable with charting and electronic medical records
Strong communication and teamwork skills
Ability to prioritize tasks and respond to changing patient needs
Basic wound care and infection control practices
