Roles & Responsibilities
- Provide skilled speech-language pathology assessments and interventions for SNF residents
- Develop individualized treatment plans addressing communication, cognition, and swallowing
- Document patient progress thoroughly and meet productivity expectations (90%)
- Collaborate with an interdisciplinary therapy team to coordinate resident care
- Follow Continuum Rehabilitation Group and facility policies and procedures
- Be present on-site to deliver therapy and ensure continuity of care
Qualifications
- Active licensure as a Speech-Language Pathologist in New Mexico or eligibility to obtain state licensure
- 1-3 years of clinical SLP experience (as indicated in the job order)
- Experience working in skilled nursing facility or long-term care settings preferred
- Strong documentation skills and ability to meet productivity requirements
- Willingness to follow facility dress code (black scrubs) and CRG policies
- Available for a 13-week contract and able to comply with a 2-week cancellation notice
Education
- Master's degree in Speech-Language Pathology preferred or equivalent graduate-level training
Knowledge & Skills
- Skilled in dysphagia assessment and management for older adults
- Proficient with evidence-based interventions for communication and cognitive-communication disorders
- Comfortable working within SNF workflows and interdisciplinary teams
- Good clinical reasoning, time management, and strong patient rapport
- Familiarity with electronic documentation systems and productivity tracking
