Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and document treatments accurately
- Monitor patient status and report changes to the care team
- Assist with activities of daily living while promoting independence
- Collaborate with nurses, therapists and support staff to deliver coordinated care
- Maintain accurate, timely charting and adhere to facility policies
- Support safe patient transfers and use proper infection control practices
Qualifications
- Current LPN license in Pennsylvania (required)
- Graduate of an accredited practical/vocational nursing program
- Current BLS/CPR certification preferred or willing to obtain
- Ability to pass required background checks and drug screening
- Strong communication skills and a team-oriented attitude
- Compassionate approach to patient care and reliable attendance
Education
- Diploma or certificate in Practical Nursing (LPN) from an accredited program
Knowledge & Skills
- Medication administration and dose documentation
- Basic assessment skills and recognition of patient changes
- Charting in electronic or paper medical records
- Infection prevention and safety procedures
- Effective communication with patients, families and clinical teams
- Time management and prioritization in a fast-paced setting
