Roles & Responsibilities
- Provide direct patient care with compassion and clinical accuracy
- Administer medications and treatments according to plan and policy
- Perform timely patient assessments and document findings clearly
- Collaborate with nurses and interdisciplinary team to coordinate care
- Monitor vital signs, report changes, and escalate concerns promptly
- Assist with procedures, wound care and activities of daily living
- Support patient education and family communication with empathy
- Maintain accurate charts and complete required documentation
Qualifications
- Active LPN license in Pennsylvania (required)
- Current BLS/CPR certification or willingness to obtain
- Comfortable working in a fast-paced facility with strong teamwork
- Prior experience in long-term care, skilled nursing, or acute settings preferred
- Clear background check and ability to meet facility onboarding requirements
Education
- LPN certificate or diploma from an accredited nursing program
- High school diploma or equivalent
Knowledge & Skills
- Strong medication administration and documentation skills
- Good patient assessment and observation abilities
- Effective communication with patients, families, and team members
- Basic wound care, IV/IVAD familiarity as applicable to the unit
- Time management and ability to prioritize patient needs
- Familiarity with electronic medical records and charting
