Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and safely
- Perform routine assessments and document changes in condition in the medical record
- Collaborate with the interdisciplinary team to create and follow individualized care plans
- Communicate clearly and empathetically with residents and families
- Monitor and report clinical changes promptly to ensure excellent outcomes
- Support a safe, clean environment and follow infection control protocols
Qualifications
- Active LPN (Licensed Practical Nurse) license in Pennsylvania (or compact/eligible where applicable)
- Current CPR/BLS certification
- Comfortable administering medications and performing basic treatments
- Strong interpersonal and communication skills; resident-focused attitude
- Previous experience in long-term care, skilled nursing, or similar settings preferred
- Ability to pass facility background checks and meet onboarding requirements
Education
- Practical Nursing diploma or equivalent from an accredited program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic nursing assessments and vital signs monitoring
- Familiarity with electronic medical records and charting
- Infection control and safety procedures
- Time management and prioritization in a busy clinical environment
- Teamwork and effective communication with care teams and families
