Roles & Responsibilities
- Provide compassionate day-to-day nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and document thoroughly
- Monitor and report changes in patient condition promptly to the care team
- Collaborate with RNs, therapists, and support staff to create and follow individualized care plans
- Assist with activities of daily living while promoting resident dignity and independence
- Maintain accurate clinical records and complete required documentation in a timely manner
- Participate in team huddles and contribute to a positive, resident-centered environment
Qualifications
- Active LPN license in Pennsylvania (or compact/state-appropriate) — required
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Previous experience in long-term care or skilled nursing preferred
- Strong communication and teamwork skills; a patient-first attitude
- Ability to pass background checks and meet facility onboarding requirements
Education
- LPN diploma or practical nursing certificate from an accredited program
- High school diploma or GED required
Knowledge & Skills
- Medication administration and basic wound care skills
- Understanding of infection control and standard precautions
- Comfortable with electronic charting and clinical documentation
- Ability to assess basic clinical signs and escalate concerns appropriately
- Time management skills to prioritize resident care effectively
