Roles & Responsibilities
- Provide compassionate bedside care and timely assessments for assigned residents
- Administer medications and treatments accurately and document per policy
- Monitor patient status, report changes, and escalate concerns to the care team
- Maintain clear, thorough electronic and paper charting to support continuity of care
- Collaborate with RNs, CNAs, and interdisciplinary team members to optimize outcomes
- Educate patients and families about care plans, medications, and safety measures
Qualifications
- Current LPN license in good standing (state-appropriate license required)
- Basic Life Support (BLS/CPR) certification required or willing to obtain
- Recent clinical experience in long-term care, skilled nursing, or similar setting preferred
- Ability to work the assigned night schedule (Tue-Thu, 6:00 PM - 6:30 AM)
- Successful background check and drug screen per facility policy
- Strong communication skills and a patient-centered mindset
Education
- LPN diploma, vocational nursing certificate, or equivalent
Knowledge & Skills
- Skilled in medication administration and documentation
- Solid understanding of vital sign monitoring and basic patient assessment
- Experience with wound care, ADLs assistance, and mobility support
- Knowledge of infection control practices and patient safety protocols
- Good clinical judgment, time management, and team communication
