Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents
• Administer medications and treatments accurately and safely
• Perform focused assessments and report changes in condition promptly
• Document care thoroughly and maintain clear communication with the care team
• Assist with activities of daily living while promoting resident dignity and independence
• Collaborate with nurses, therapists, and support staff to coordinate care
• Support admission, transfer, and discharge processes as needed
• Participate in facility safety and infection control practices

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania or compact-state eligibility
• Completion of an accredited LPN/LVN program
• Current Basic Life Support (BLS) or CPR certification preferred
• Ability to pass background checks and meet facility immunization requirements
• Comfortable working in a long-term/skilled nursing environment and providing hands-on care
• Strong communication skills and a collaborative attitude

Education

• Graduation from an accredited LPN/LVN program; active LPN license required

Knowledge & Skills

• Clinical knowledge of medication administration and wound care basics
• Skilled in performing focused nursing assessments and documenting findings
• Familiarity with electronic or paper charting and chart maintenance
• Ability to prioritize tasks in a fast-paced care setting
• Compassionate bedside manner and strong teamwork skills
• Understanding of infection control and resident safety practices
