Roles & Responsibilities

• Provide direct, compassionate nursing care to residents in a skilled nursing environment
• Administer medications and treatments accurately and document all care thoroughly
• Monitor resident condition and report changes promptly to the interdisciplinary team
• Assist with activities of daily living and promote resident comfort and dignity
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Maintain accurate and timely clinical documentation in facility records
• Support a safe, clean environment and adhere to infection control and facility policies

Qualifications

• Current LPN (Licensed Practical Nurse) license in Pennsylvania (required)
• Current Basic Life Support (BLS) certification or willingness to obtain prior to start
• Prior experience in long-term care or skilled nursing preferred but enthusiastic new grads considered
• Strong communication skills and a teamwork mindset
• Ability to pass background check and meet facility onboarding requirements

Education

• Graduate of an accredited practical nursing program or equivalent LPN training

Knowledge & Skills

• Medication administration and resident assessment skills
• Familiarity with wound care basics and assistance with ADLs
• Comfortable with electronic charting or willingness to learn facility electronic health records
• Good time management, clinical judgment, and documentation practices
• Patient-centered approach and ability to work collaboratively with interdisciplinary teams
