Roles & Responsibilities
- Provide compassionate, high-quality direct patient care tailored to each resident
- Administer medications and treatments safely and accurately
- Monitor and document patient condition, reporting changes promptly to RNs and providers
- Collaborate with interdisciplinary team members to support resident care plans
- Assist with activities of daily living while promoting dignity and independence
- Educate residents and families on care instructions and health promotion
- Support facility initiatives for infection control, safety, and regulatory compliance
Qualifications
- Current LPN (Licensed Practical Nurse) license—eligible to practice in Pennsylvania
- Graduate of an accredited practical nursing program or equivalent diploma
- Basic Life Support (BLS/CPR) certification required or willing to obtain
- Compassionate patient-focus and strong communication skills
- Ability to pass required background checks and health screenings
- Previous experience in long-term care or skilled nursing preferred but not required
Education
- Diploma or certificate from an accredited LPN practical nursing program
- High school diploma or GED
Knowledge & Skills
- Strong clinical assessment skills and reliable medication administration
- Accurate charting and familiarity with electronic health records
- Understanding of infection control practices and patient safety
- Team-oriented mindset with clear verbal and written communication
- Good time management and the ability to prioritize tasks in a busy environment
