Roles & Responsibilities

• Provide compassionate direct patient care, monitoring residents and addressing needs promptly
• Administer medications and treatments safely and accurately according to orders
• Document patient status, interventions, and outcomes clearly in the charting system
• Collaborate with interdisciplinary team members to create and follow personalized care plans
• Observe and report changes in residents’ conditions to nursing leadership and providers
• Support residents with activities of daily living while promoting dignity and independence

Qualifications

• Active LPN license in Pennsylvania or ability to obtain licensure in PA
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable administering medications and performing routine nursing procedures
• Strong communication skills and a compassionate, patient-first mindset
• Previous experience in long-term care or skilled nursing preferred but new grads welcome

Education

• Certificate or diploma from an accredited Practical Nursing program (LPN)

Knowledge & Skills

• Solid knowledge of nursing fundamentals, medication administration, and infection control
• Familiarity with electronic medical records and accurate documentation practices
• Ability to prioritize tasks in a busy care environment and respond calmly in emergencies
• Good interpersonal skills for working with residents, families, and the care team
• Attention to detail and a commitment to quality, resident-centered care
